Objective: The primary aim of the study is to estimate the benefit of annual mammographic surveillance in terms of reducing late stage breast cancer and mortality from the disease in women aged 40-49 (ie below the age limit of the national screening programme) with a significant family history of breast cancer. This will be done by ascertaining cancers and their prognostic attributes in women undergoing this surveillance, estimation of the expected mortality from these, and comparison of the mortality expected in the absence of the surveillance. With the high media profile of breast cancer there is increasing awareness of family history of the disease in the population and a subgroup with a sufficient family history to confer a trebling of risk is increasingly presenting to primary care and genetics services. There is an urgent need to develop and evaluate surveillance and preventive services to manage this subpopulation.
